A health insurance company has a right to recover its payments for your medical bills from both you and the person who caused the accident. This is called subrogation.
If I don’t think the settlement will be enough to be fair to both you and your health insurance company – or if I think the health insurance company will be unreasonable – I sue the health insurance company as part of the lawsuit. It is a judgment call.
This makes them get a lawyer, and acknowledge whatever weaknesses there are in their case. It also forces them to confront the reality that the jury may not be as generous with them as it will be with an individual. This leads to settlements.
